Your feedback plays an important role in how West Street Surgery operates, so please do not miss this opportunity to have your say.

The Patient Participation Group (PPG) would like to invite all patients to join the group and, if willing, to become a member of the participating committee.

The PPG asks you to give your views on how the surgery practice is doing now. Do you see improvements, the same or is performance falling?

Please complete as many questions as you are able. The information you submit is entirely anonymous.

Monday to Friday – 08:00-13:00, 14:00-18:30 (closed for lunch/training 13:00 – 14:00 daily)

Extended Access Mondays – 18:30-20:00

Extended Access Saturdays – 08:30-12:30 (by special appointment)
